Botany Home Care is an independent Sydney disability support provider for self-managed and plan-managed NDIS participants. We provide practical, respectful support at home and in the community, shaped around each participant’s goals, preferences and routine.

Botany Home Care is a Sydney-based disability support business focused on responsive, personal service. We take the time to understand each participant and work toward consistent, respectful support relationships.
How We Work
Support begins with listening. Before services commence, we clarify your goals, routine, communication preferences, boundaries, schedule and responsibilities so everyone understands what has been agreed.
Who We Support
We currently operate as an unregistered NDIS provider and support self-managed and plan-managed participants. We cannot currently provide NDIS-funded supports to participants whose relevant funding is NDIA-managed.
From Referral to Ongoing Support
A clear onboarding process helps participants, families and referral partners understand what happens at every stage.

The participant, family member, support coordinator or other authorised person contacts us.

Initial Conversation
We discuss support needs, goals, location, availability and plan-management arrangements.

Intake and Assessment
With consent, we collect the information required to understand support needs, preferences, risks and communication requirements.

Agreement and Support Plan
We document agreed services, rates, schedules, responsibilities, cancellation terms and participant preferences.

Support Commences
Support begins according to the agreed schedule, with clear communication and suitable worker arrangements.

Review and Feedback
We seek feedback and review services when goals, circumstances or support needs change.
Information for Participants and Referrers
Clear answers about eligibility, referrals, service arrangements and what happens before support begins.
We currently work with self-managed and plan-managed NDIS participants. As an unregistered provider, we cannot deliver NDIS-funded supports charged to NDIA-managed funding. If you are uncertain how a participant’s funding is managed, contact us before submitting a referral.
Depending on participant needs, worker suitability and current capacity, supports may include assistance with daily living, personal care, community participation, social support, appointments and transport assistance. The agreed scope of support is confirmed during intake and documented before services commence.
A referral should include the participant’s consent, suburb, plan-management type, goals, requested supports, preferred schedule, communication requirements and relevant safety information. Detailed or unnecessary sensitive documents are not required with the initial enquiry.
Before support commences, we explain the proposed schedule, rates, travel arrangements, cancellation terms and responsibilities. These details are recorded in a written service agreement for the participant or authorised representative to review and accept.
Communication preferences and authorised contacts are confirmed during intake. Participant information is only shared with consent, where required for safe service delivery, or where disclosure is required by law. Feedback and complaints can be raised without affecting a participant’s right to respectful support.
We review the referral, confirm funding type, capacity and suitability, and arrange an initial conversation. If both parties wish to proceed, we complete intake and risk assessment, agree on the support plan and service agreement, organise a suitable worker and confirm the commencement date.
